Description: As a Home from Hospital volunteer, you will support older people 65+ in Bolton post-hospital discharge and older people 50+ to avoid an admission to hospital with practical, emotional, and welfare support, enabling safe, confident recovery at home. What you'll be doing * Visiting clients in their homes, following an agreed support plan (typically up to 6 weeks). * Providing welfare support, noting safety or well-being concerns. * Reporting any concerns promptly to the Service Coordinator or Team Leader. Help with: * Home safety and warmth checks * Prescription collections * Shopping for essentials * Light housework (e.g., tidying, dishes, putting away shopping) * Sorting post or bills * Applying for Blue Badges * Encouragement with exercises (OT/physio) * Attending or helping to book medical appointments Please note this role does not involve any personal care.
