Greater Manchester Population Health Plan Review

A recent draft report has been produced by Greater Manchester Health and Care Board around Greater Manchester Population Health Plan Review.  
 

The report provides an overview of activities undertaken in 2017/2018 towards the priorities set out in the Greater Manchester (GM) Population Health Plan.

The report will cover the following areas:-

  • Work to date to agree allocation of Population Health Transformation Fund monies against a number of strategic business cases.
  • Progress so far in terms of implementation of a number of early programmes of work as part of the plan.
  • A forward look at future planned activities.
